Our team assesses every area of the home, identifies hazards, installs necessary equipment, and ensures the living space supports a smooth, confident recovery.

50-Point Home Safety & Preparation Evaluation
Our certified team conducts a comprehensive room-by-room assessment before your loved one comes home. Here is what the evaluation covers.
Fall Risk Assessment: We walk every room, hallway, and transition area to identify trip hazards, inadequate lighting, loose flooring, and unsafe surfaces. Grab bars, non-slip strips, and threshold modifications are installed where needed.
Equipment Installation: Hospital beds, transfer aids, shower chairs, raised toilet seats, bedside commodes, and any other recovery equipment are positioned and secured before the patient arrives home.
Accessibility Modifications: Ramp installation, doorway widening, stair lift assessment, and clear pathways for wheelchair or walker use. We make sure the patient can move safely through every part of the home.
Kitchen & Bathroom Preparation: Water temperature settings, grab bar placement, faucet accessibility, medication storage, and removal of hazards from high-traffic areas. Both rooms are prepared for limited mobility use.
